The Match Number will be on the GDR. It can also be found in Arbiter in the “Custom Game ID (Other)” field.
In addition to the online report, a report of ALL misconduct (player and/or bench personnel) and serious injuries in a game must be sent by email to the appropriate division commissioner with copy to your assignor AND:
